Across Mid and South Essex, Havens Hospices, along with Farleigh Hospice and St. Luke’s Hospice (the Hospice Collaborative) have launched a new pathway to enable quick access for patients requiring care as their condition is rapidly deteriorating, and they have a primary health need.
This new pathway will be managed by the Hospice Rapid Access service, which will then ensure that patients receive community-based care in the most appropriate place for them, for example, their home, inpatient hospice beds or a care home.
