If a patient’s wish is to die in the comfort and familiarity of their own home, Fair Havens Hospice at Home can help.
Our specialist team of Palliative Care Nurses and Nursing Assistants will provide the care and compassion someone at the end of their life deserves, as well as supporting their family.
They will support discharges from hospital to home, offer planned respite 24 hours a day and provide care up until the final moments of life.
The services of Fair Havens Hospice, including our Hospice at Home service, can provide care for people from the age of 19 who have been diagnosed with a complex or incurable condition. They must be registered with a GP within Southend, Castle Point or Rochford.
Initial contact may be made either by a patient or carer, or any professional directly involved in the patient’s care. Anyone making a referral must have the consent of the patient.
We will agree a Support Plan with you. This will enable you to identify your individual needs and goals. Together, we will develop a plan to help you in achieving your goals. This may involve you attending one day a week for six to 12 weeks or accessing our services on a seasonable basis, for example; out-patient appointment with Clinical Nurse Specialist, complementary therapy treatments or counselling.
