About Our Care

Many people wish to stay in the comfort and familiarity of their own home, and this is often possible with the right support. 

Fair Havens Community Team can provide the specialist care and support you and your loved ones need to manage at home when you are living with an incurable condition, and make sure all of your needs are met, and you are comfortable, when the end of life is in sight. 

Our specialist team of Nurses and Nursing Assistants provide personalised, compassionate care, which is always based on your choices and wishes. 

Fair Havens Community Team will also support you or your loved one returning home from hospital, they can offer planned and urgent respite support in the day and overnight - because carers need to rest and recharge.  The team is there for you 24 hours a day, at the end of the phone, for advice and support, or to arrange additional home visits, or visits to Fair Havens Hub or In Patient Unit.

 Fair Havens, including our Communtiy Team, can provide care for people from the age of 19 who have been diagnosed with a complex or incurable condition. They must be registered with a GP within Southend, Castle Point or Rochford.

Initial contact may be made either by a patient or carer, or any professional directly involved in the patient’s care. Anyone making a referral must have the consent of the patient.
