Transition Support Worker

We have an exciting opportunity for a Transition Support Worker to join an expanding wellbeing team, part of Havens Hospices. It may be suitable for youth worker, family support worker or healthcare assistant who is looking to develop or enhancing skills in transition support work.

The main duties of this role include working with patients aged 14-25 with life-limiting conditions to offer guidance and support while transitioning from children’s services to adult services, raising aspirations and to help remove barriers to successful transition, while promoting autonomy and independence.

The successful candidate will have experience of working within a children and young person related discipline, NVQ level 3 in Health and Social Care or equivalent, experience of working therapeutically one on one or within a small group and an in-depth knowledge of safeguarding. A keen ability to engage with children, young adults and families in the process of assessing and identifying needs is essential.
