Hospice at Home Deputy Sister

We are currently recruiting a Deputy Sister to join our established Adults Hospice at Home Team, ensuring a holistic package of care is provided to patients and families within their own home. The successful applicant will be pivotal in assisting the Hospice at Home Sister in the day to day running of the service.

At Havens Hospices we care for children, young people and adults with complex or incurable conditions. Our specialist Care Teams can support them and their family living throughout illness, death and bereavement, in the comfort of their own home and through our hospice services, Fair Havens, The J’s and Little Havens.

Key responsibilities

  • Manage Community Nurses and Nursing Assistants during shift to enable patients care needs to be met.
  • Contribute to the planning, delivery and evaluation of interventions and treatments with the Hospice at Home Sister and palliative care nurses/consultant.
  • To ensure that, as far as possible, family and / or carers are included in the care provided.
  • To liaise with other professionals working within the primary and secondary care settings, regarding patient care when required.
  • To attend, participate and present patients in weekly multidisciplinary team meetings where appropriate.
  • Maintain and promote therapeutic working relationships with both internal and external health care professionals and organisations including GP’s, EPUT, SUH and CCG teams.
