Family Support Worker

Contract: Part Time - 15 hours per week
Location: Fair Havens

We are currently recruiting a Family Support Worker to join our established Wellbeing Team, to provide support for patients, their carers and wider family members. The successful applicant will be pivotal in delivering an empowering, flexible and integrative style of person-centered care, to adults, families, children, and young people.

Candidates will have:

  • Experience of working with people and handling difficult situations.
  • Working and supporting families pre and post bereavement
  • Intermediate keyboard and IT skills, working with Word, Excel and outlook, powerpoint
  • Numeracy skills
  • Excellent customer service
  • Willingness to learn and develop new skills
  • Ability to communicate sensitively and diplomatically with a wide range of people

At Havens Hospices we care for children, young people and adults with complex or incurable conditions. Our specialist Care Teams can support them and their family living throughout illness, death and bereavement, in the comfort of their own home and through our hospice services, Fair Havens, The J’s and Little Havens.

Key Responsibilities

  • To work as a key member of the Wellbeing Team to ensure high quality service delivery to all patients, carers and wider family members in an ethical and effective manner in line with organisational and professional standards.
  • To be responsible for a family support caseload as defined in conjunction with and approved by the Hospice Social workers and / or Head of Welfare and Wellbeing
  • To carry out home visits and complete Holistic needs assessments.
  • To plan and implement appropriate sessions for identified areas of support
  • Communicate effectively with members of the multidisciplinary team, District Nurse, GP, Social Services and other organisations
