If a hospital diagnosis has been given which means a person’s life will be limited or threatened by illness, then Fair Havens Hospice may be able to help through its various care services. These include the In Patient Unit, Day Hospice and Hospice at Home.
how can I refer myself/someone i’m caring for to fair havens hospice?
Although a referral can be made direct to the hospice by a patient or carer, we do recommend that a professional involved in the patient’s care make the initial contact. This way Fair Havens Hospice has all the medical information needed to consider the referral.
Anyone making a referral must have the consent of the patient.
Please ask your GP or a healthcare professional to complete the Specialist Palliative Care Referral Form and fax on 01702 437009 back to Fair Havens Hospice.

criteria for referral
- Live within the Southend, Castle Point and Rochford Primary Care Trust areas and who are registered with a GP in the area
- Are affected by a life-limiting/life-threatening illness and require symptom control, respite care or end of life care
- Are over the age of 19 years.
Patients between the ages of 16-19 may be considered for admission if they are in the process of transitioning from children’s services to adult services and are already known to Little Havens Hospice or other adult palliative care services.
Referrals to the In Patient Unit may be made by any healthcare professional. However, it must be with the patient’s consent.
what other care can fair havens hospice offer?
The charity’s Community Hospice Service can offer care within a patient’s own home, telephone support or regular visits to our Day Hospice.
