Who we can help

Patients should be a resident within the South East Essex PCT area for Fair Havens Hospice. Patients from outside the area can be considered for admission if they move into the district to stay with relatives/friends and are registered with a South East Essex GP. All admissions for patients normally residing outside of the catchment area (Southend, Castle Point, Rohford) must be with the authorisation of either the Head of Patient Services or the Medical Consultant.

To be considered for admission patients must have a progressive disease where the prognosis is known to be life limiting or life threatening. Both patient and carer must be aware of the referral and be accepting of the admission.

Referrals must be made on the Single Referral Form for Palliative Care Services (please see download to the right) and faxed or e-mailed to the department. Telephone referrals can be accepted from the Fair Havens Community Macmillan Palliative Care Team and Fair Havens Hospice at Home, but must be followed up with a completed referral form or further information within 24 hours.

The single referral form is freely available at all health care establishments locally. However, forms can be faxed to organisations, or individuals, that do not have access. Information contained on the referral form remains confidential to the Fair Havens care services.
