How to access our services

Referrals to any of the services of Fair Havens Hospice can be made by the patient, a member of their family or a healthcare professional involved in the patient's care such as:

• A Macmillan Nurse
• District Nurse
• GP
• Hospital Consultant
• Hospital Ward
• Social Worker

To find out if our care would be suitable for you or to enquire about making a referral, please speak to a health professional involved in your care or find out for yourself.
