Hospice at Home

We care for patients who are in the advanced stages of a life-limiting illness, supporting them at home if this is their wish.

Hospice at Home offers planned respite care during the day and night, plus a 24/7 advice and support service by telephone. Our team can provide emergency intervention at times of crisis to prevent unwanted and unnecessary admissions to hospital and can also help to put care plans in place to assist in the discharge from hospital.

The team works together with the Fair Havens In Patient Unit and Day Care Centre, Fair Havens Macmillan Nurses, the Primary Care Team and the acute trust, and Essex Social Services to enable patients to achieve their preferred place of care and/or death, and support those who matter to them.

Who we can help

Patients who are referred to the team must have a recognised life-limiting or life-threatening illness, although their illness does not have to be imminently terminal.

Planned respite is reviewed regularly with everyone involved, including health and social care professionals. Anyone can refer to Fair Havens Hospice at Home although the majority of our referrals come from heath and social care professionals.

Patients referred for this service may include someone who is:

  • Suffering from a recognised life-limiting or life-threatening illness and whose family/carer is finding it difficult to manage due to a crisis
  • The team can also provide support where the carer has been taken ill, or the patient has no carer
  • In need of planned respite to support the patient and those who care for them so that they may continue to care for their loved one
  • Believed to be close to death and the family or those who matter to them are anxious or exhausted, or the patient lives alone

Our Team Fair Havens Hospice at Home is a partnership between Havens Hospices and SPDNS Nurse Care CIC. The Fair Havens Hospice at Home team include nurses who have significant experience and are passionate about delivering care of the highest standard, personalised to meet the needs of each individual and their family/carer.

There is continued support for families and carers following bereavement and we can refer families to the specialist Fair Havens Family Support Team.
